Choosing Healthy Blue

After you are notified you qualify for Medicaid, the second step is you need to pick a health plan to provide your health care.

Step 2 — Picking Your Health Plan

Once you are determined eligible for Healthy Connections Medicaid, you will receive a Healthy Connections Medicaid Enrollment Packet, which will include a Health Plan and Doctor Selection Form. Use this form to choose a health plan and doctor, or primary care provider (PCP), for each person in your family that qualifies for Medicaid. Then return the form to South Carolina Healthy Connections Choices using the postage-paid envelope provided in your Enrollment Packet.

As a new Healthy Connections Medicaid beneficiary, you must pick a health plan within 60 days. If you do not choose a health plan, South Carolina Healthy Connections Choices will assign a plan for you.

How to choose Healthy Blue?

Choose Healthy Blue using any one of the following options:

1. Online

You’ll need:

  • Your Healthy Connections Medicaid member ID.
  • Healthy Connections Choices Medicaid PIN.
  • Date of birth.
    Find your member ID and PIN in your letters from Healthy Connections Choices.

Go to the Healthy Connections Choices website

  1. Select Enroll Now
  2. Enter your Healthy Connections Medicaid member ID
  3. Enter your Healthy Connections Choices Medicaid PIN
  4. Enter your Date of Birth
  5. Select Continue
  6. Follow the step-by-step instructions and choose Healthy Blue

2. By phone

Call the Healthy Connections Choices Customer Service Center at 1-877-552-4642 (TTY 1-877-552-4670) Monday through Friday from 8 a.m. to 6 p.m. Eastern time (except for South Carolina state holidays).

Tell them you choose Healthy Blue.
Translation services are available.

3. By mail or fax

Choose Healthy Blue on the Health Plan and Doctor Selection Form found in your Healthy Connections Medicaid New Enrollment Packet. Mail or fax the completed form:

To learn about Healthy Blue, call the Healthy Blue Prospective Member Line at 1-800-574-8864 Monday through Friday from 12 to 7 p.m. Eastern time. Medicaid application assistance inquiries will be referred to the Healthy Connections Customer Service Center.

Choose your Healthy Blue primary care provider (PCP)/doctor

Once you choose Healthy Blue as your health plan, choose a doctor or PCP for each family member that qualifies for Medicaid. Your PCP/doctor will get to know you and your health needs and coordinate all of your health care. If you don’t choose a doctor or PCP, Healthy Blue will assign one for you.

Do one of the following to view a list of our Healthy Blue doctors or PCPs:

View our doctor finder — Find a Doctor.
Our doctor finder is the most up-to-date list of the different types of doctors in our health plan. To choose a PCP/doctor, select either Basic Search or Advanced Search.

For a Basic Search:

  1. Select Basic Search as a simple way to look for a PCP/doctor.
  2. Enter your ZIP Code.
  3. Choose a Specialty Type. You can choose from pediatrics, family medicine, internal medicine and more.
  4. Select a Language.
  5. Select Search.
  6. Your search results will appear. Pick a PCP/doctor from the list.

For an Advanced Search:

  1. Select Advanced Search as a full way to look for a PCP/doctor. An advanced search is not required, but it will help you create a specific list of PCP/doctor options that closely matches your preferences.  Enter or select as many of the requested details as possible for the best search results.
  2. Select Search.
  3. Your search results will appear. Pick a PCP/doctor from the list.

Get a printed version of our Healthy Blue doctor finder. Go to Find a Doctor to print a copy.

  1. Follow the Basic Search or Advanced Search steps above.
  2. Select View/Print Directory.
  3. Select View Whole Directory.
  4. Select the Printer icon to print your provider directory.

Call us to ask for a FREE, printed provider directory — Our Healthy Blue Customer Care Center can mail a copy to you, and we can help you choose a PCP/doctor.

Once you choose a doctor for yourself and each member of your family, tell us!

    1. Your Healthy Connections Medicaid member ID.
    2. Your Healthy Connections Choices  PIN.
    3. Date of birth.
      (You can find your member ID and PIN in your letters from Healthy Connections Choices.)
  4. Go here on the Healthy Connections Choices website:
    1. Select Enroll Now.
    2. Enter your Healthy Connections Medicaid member ID.
    3. Enter your Healthy Connections Choices  PIN.
    4. Enter your date of birth.
    5. Select Continue.
    6. Follow the step-by-step instructions to choose a PCP.

If you don’t choose a PCP/doctor for each member of your family that qualifies for Medicaid, Healthy Blue will assign one to you.

What happens after you choose Healthy Blue?

As a new Healthy Blue member, we’ll send you a Healthy Blue New Member Welcome Kit and ID card within 15 calendar days of choosing our health plan. It will include:

  • New Member Welcome Letter
  • Evidence of Coverage (EOC) (Member Handbook)
  • Primary Care Provider (PCP) Selection Form
  • Notice of Privacy Practices

You’ll also receive two member ID cards — one from South Carolina Healthy Connections and one from Healthy Blue. Keep both member ID cards with you at all times. You’ll need to show them each time you get health care services. Don’t let anyone else use your ID cards.

Some Reminders

  • The name and phone number of your PCP/doctor will be printed on the front of your Healthy Blue ID card. Make sure the doctor on it is the one you want for as your PCP/doctor. If you want a different PCP/doctor, let us know right away by calling our Customer Care Center.
  • Set up your first health exam with your doctor or PCP right away. All new Healthy Blue members should see their PCPs/doctors within 90 days of joining.

How to renew your Medicaid eligibility each year?

Once you have had Medicaid, every year, the South Carolina Healthy Connections Medicaid program will check to see if you still qualify for Medicaid. They will send you a Healthy Connections renewal packet, which will ask you for specific information. This packet is very important, so do not throw it away.
