Renew My Eligibility

You will need to supply information to the South Carolina Department of Health and Human Services (SCDHHS) and have them determine if you are still eligible to receive Medicaid benefits each year.

Step 3 – Renew Today

Once you have had Medicaid, every year, the South Carolina Healthy Connections Medicaid program will check to see if you still qualify for Medicaid.  They will send you a Healthy Connections renewal packet, which will ask you for specific information. This packet is very important, so do not throw it away.

Please fill out the requested information in your Healthy Connections renewal packet. Then turn-in your completed renewal information by the deadline appearing in your packet.

You may submit your completed Healthy Connections renewal information using any one of the following options.

  1. In Person:

If you do not fill out the renewal packet and respond by the deadline in your packet, Healthy Connections may stop your Medicaid benefits.

How to keep Healthy Blue as your health plan

Staying with Healthy Blue is simple.

Once you have submitted your renewal packet and the state determines you are still eligible to be enrolled in a managed care plan, Healthy Connections Choices will mail you a letter prior to your managed care anniversary each year.  If you are a current Healthy Blue member and want to remain with Healthy Blue, you do not have to do anything. Healthy Connections Choices  will automatically keep you enrolled with Healthy Blue.

How to select Healthy Blue as your health plan

You can switch to Healthy Blue. 

Once you have submitted your renewal packet and the state determines you are still eligible to be enrolled in a managed care plan, Healthy Connections Choices will mail you a letter prior to your managed care anniversary each year.  If you are not a current Healthy Blue member and want to switch to Healthy Blue, you must select Healthy Blue as your health plan within 60 days of your managed care anniversary and Healthy Connections Choices  make the change for you. You can select Healthy Blue as your Health Plan by doing one of the following:

  • Go to the Healthy Connections Choices website.  You must select Healthy Blue when asked which health plan you want. 
  • Call the Healthy Connections Choices Customer Service Center and tell them you would like to switch to Healthy Blue.

    All health plan selection requests will be referred to Healthy Connections Choices.
Can you stay with your current primary care provider (PCP)/doctor after you renew; or do you have to choose a new PCP/doctor?

You can keep your current PCP/doctor if he or she is still in-network with Healthy Blue. Or you can also choose a new PCP/doctor. 

To view a list of our Healthy Blue providers and doctors, use any one of these options.

Go to www.HealthyBlueSC.com and select Find a Doctor.

It is the most up-to-date list of the different types of doctors in our health plan. From our online Doctor Finder, you can select Basic or Advanced Search to find a PCP/doctor.

For a Basic Search:

  1. Select Basic Search as a simple way to look for a PCP/doctor.
  2. Enter your ZIP Code.
  3. Choose a Specialty Type. You can choose from Pediatrics, Family Medicine, Internal Medicine and more.
  4. Select a Language.
  5. Select Search.
  6. Your search results will appear. Pick a PCP/doctor from the list.

For an Advanced Search:

  1. Select Advanced Search as a full way to look for a PCP/doctor. An advanced search is not required, but it will help you create a specific list of PCP/doctor options that closely matches your preferences.  Enter or select as many of the requested details as possible for the best search results.
  2. Select Search.
  3. Your search results will appear. Pick a PCP/doctor from the list.

Go to Find a Doctor to print a copy.

  1. Follow the Basic Search or Advanced Search steps above.
  2. Select View/Print Directory.
  3. Select View Whole Directory.
  4. Select the Printer icon to print your provider directory.

Call us to ask for a FREE, printed provider directory.

Our Healthy Blue Customer Care Center can mail a copy to you, and we can help you choose a PCP/doctor.

If you choose a new PCP/doctor:

  • Healthy Blue  will send you a new member ID card in the mail. It will have the name and phone number of your new PCP/doctor on the front of the card.
  • Remember to keep your Healthy Blue and your South Carolina Healthy Connections ID cards with you at all times. You’ll need to show them each time you get health care services. Don’t let anyone else use your ID cards.
  • Make sure the PCP/doctor on the front of your Healthy Blue  ID card is the one you chose. If it’s incorrect, tell us right away by calling our Customer Care Center at 1-866-781-5094 (TTY 1-866-773-9634), Monday through Friday from 8 a.m. to 6 p.m. Eastern time.
  • Set up your first health exam with your new PCP/doctor right away.
Will you receive notification once SCDHHS determines that you are still eligible for Medicaid?

SCDHHS will send you a letter about the status of your Medicaid eligibility every year. The letter could be a:

  • Closure notice — this letter tells you your Medicaid coverage is not reapproved.
  • Continuation notice — this letter tells you if you still have Medicaid benefits.
  • Change in benefits notice — this letter tells you if something has changed with your Medicaid benefits. For example, if there is a change in type of benefits, such as full Medicaid to Family Planning benefits, only.
