Renew your South Carolina Medicaid benefits

You need to renew your benefits every 12 months to keep them. If you don’t renew on time, you could lose your benefits.
But don’t worry! Just follow these easy steps to renew your benefits now.
1. Keep an eye out for a notice in the mail.
You’ll get a Healthy Connections Annual Review form from the South Carolina Department of Health and Human Services (SCDHHS) when it’s time to renew your benefits. They’ll check to see if you still qualify for Medicaid.
2. Complete your form and gather documents.
Completely fill out the form and gather any documents SCDHHS asks for.
3. Send your information back.
You can submit your completed Healthy Connections renewal information:
- In Person: Go to a Healthy Connections County Office Monday through Friday from 8:30 a.m. to 5 p.m. Eastern time *This option may not be available at this time. Please call Healthy Connections if your local office is closed.
