There are no copays for preventive and urgent care services. Your benefits include:
- Primary care provider (PCP) visits
- Immunizations and wellness visits
- EPSDT (early and periodic screening, diagnosis and treatment) services for members under 21
- Specialist visits
- Urgent and emergency care
- Hospital care
- Lab and X-ray services
- Pregnancy services
- Family planning
- Chiropractic services
- Physical and occupational therapy
- Speech and hearing services
- Behavioral health care (mental and substance abuse services)
- Home health care services
- Disease management for asthma, diabetes and more

Find a Doctor
Registered nurses are available anytime, day or night. They can:
- Advise you on where to go when you need care.
- Help you with any health questions you have.
Call the 24-Hour Nurseline 24 hours a day, seven days a week at 1-866-577-9710 (TTY 1-800-368-4424).
Dental benefits are provided through DentaQuest.
Members age 21 and older can get up to $750 per year to use for covered dental services like:
- Oral exams
- Cleanings
- X-rays
- Extractions
- Fillings
- Medically necessary emergency procedures from an oral surgeon
Members under 21 years can get:
- Diagnostic, preventive, restorative and surgical benefits
- Routine dental care
- Oral exams
- Cleanings
- X-rays
- Fluoride treatments
There are no copays for emergency services. There’s a $3.40 copay for other dental services.

Vision benefits are provided by Vision Service Plan (VSP). Vision services include:
Members under 21 years of age
- Preventive screenings
- Eye exam once a year
- Glasses (frames and lenses) once a year
Members 21 years of age and older
- Eye exam once a year
- Glasses (frames and lenses) once every two years
There’s no copay for routine vision services. There’s a $3.30 copay for members 19 years of age and older who see an eye doctor for medical reasons.

Your benefits include a wide range of prescription and over-the-counter (OTC) medicines. We work with CarelonRx to provide these benefits. There is a $3.40 copay for members 19 years of age and older. Members under age 19 and pregnant women do not have copays. Visit the Pharmacy page to find a pharmacy near you and check if your medicine is covered.
CarelonRx is an independent company providing pharmacy benefit management services on behalf of BlueChoice HealthPlan.
You can get no-cost rides to your medical appointments. You or your provider should call to schedule your ride at least three business days before your appointment.

Pregnant members and new parents can receive a:
- Free Sam’s Club membership.
- Gift card to Barnes & Noble to grow your baby’s library.
- Free infant car seat for going to prenatal visits.
- Free circumcision for boys up to 1 year old.
- Free case of diapers for members newborn–15 months with well-child checkup.
- Healthy Rewards reloadable debit card to use at certain retail stores for going to prenatal and well-child checkups.
- Free electric breast pump.
- Free prenatal program with pharmacy and nutritional counseling.
- Free referrals for in-home nurse care before and after your baby is born.
Members in case management can receive:
- Free Uber gift card or free annual oil change.
Members with diabetes can receive:
- Two months of fresh fruits and vegetables delivered to their home.
- An Asthma Catalog worth up to $200.

Referrals
A referral is when your primary care provider (PCP) sends you to another provider for care. This care is often from a specialist. As a Healthy Blue member, you don’t need a referral to see a specialist in your plan.
Preapprovals
You may need preapproval for certain services. Preapproval is when your doctor asks us to approve a service before you receive it. Your PCP will work with us to get the approval you need.
You do not need an approval from your PCP for these types of care:
- Primary care services
- Women’s health care services
- Family planning services
- Children’s screening services
- Services provided at local health departments
- Emergency care
- Urgent care
Preapproval is needed for:
- Audiology (hearing services)
- Behavioral health and substance use services
- Durable medical equipment (DME) and disposable supplies
- Home health care
- Hospital inpatient services
- Institutional long-term care facilities and nursing homes
- Some lab and X-ray services
- Some transportation services
- Certain prescriptions and medications
- Some therapy services such as:
- Physical therapy
- Occupational therapy
- Speech therapy
- Transplant services
- Care from a provider who doesn’t work with your plan
