It’s About Healthy Living

Care Management Director's Message

Welcome to our spring and summer issue! As the weather warms up, it's the perfect time to enjoy the outdoors and all the exciting activities this season has to offer. Jennider Gantt

In this Healthy Living issue, we feature articles to help you and your family stay healthy and active. From tips for planning seasonal fun and an easy, kid-friendly recipe to following Coach Blue's game plan, we've got you covered.

Plus, learn how to watch your weight and manage or prevent diabetes and obesity. Let's make this spring and summer your healthiest yet.

Care Management Program

One of our goals at Healthy Blue is to give each member information and tools to stay healthy. That’s why we have our Care Management program, which gives our members focused health care information, useful resources and long-lasting support.

It's like having a team of helpers who make sure you get the right care at the right time. This way, you can stay as healthy as possible. We have three special programs to help our members manage their weight, prevent diabetes and stay healthy.

Weight Management

Members can get real-time digital help from health coaches by joining our weight management programs through the My Health Planner℠ app. We can check in with you daily or weekly during a year-long program or you can get help through a targeted 90-day weight loss program.

Diabetes Prevention and Management

We know improving your diet and exercise habits can be tough, but slow and steady changes can last a lifetime. Our diabetes program offers support to help members eat healthier. You can meet with a dietitian to learn how to cook meals that support a better way to eat. Plus, after coaching, healthy food boxes are offered to members.

Preventive Care

It’s more about healthy living than what you weigh. That’s why we offer preventive care such as tests, exams, screenings and vaccines to help members stay well. Take a look at our Preventive Health Guidelines to find out what steps you and your family should take to stay healthy.

Taking care of you and your baby is super important, too. Check out what your Healthy Blue benefits cover and what special programs we have for pregnant women and new parents. Plus earn free gift cards for doing things such as going to well-baby and adult checkups and for getting certain health screenings.

Community Health Workers

In this newsletter, we also share details about our Community Health program and the team of frontline workers who help our members find ways to stay healthy.

Email our community health workers, licensed social workers and health promotion coordinators at HBCommunityReferrals@HealthyBlueSC.com. They can provide one-on-one support and quick help when and where you need it. Reach out to us today.

At Healthy Blue, we aim to help our members make lasting changes. Please join us in this effort to help you find ways to stay healthy!
